Frontier-class models (opus/fable/sol/max-class ids) are doctor-invalid outside implementer_high/reviewer_security/engine strong+review without the allow_costly_cheap_lanes waiver (smoke-covered, bash+dash); the 4.1 inline fallback now names its lane's configured model instead of a bare subagent call that would inherit the orchestrator's model; deploy-relevant lanes use non-closing PR references and close issues only after deploy_verify.
